    Going on school camp can be an exciting adventure for children. Sometimes parents are worried that the school will not know how to look after their child's diabetes care needs whilst on camp. A dedicated school camp plan prepared by your diabetes team will help the teachers plan how best to support your child. Preparing a plan takes time so听requests must be made at least 4 weeks before needed by the school.

    Please note we aim to begin developing your child鈥檚 camp plan 7-10 days prior to their camp. For us to be able to complete the plan it is important that you have provided all necessary data in the request form and have up to date continuous glucose monitoring (CGM) data available to review online. If we do not have access to all the necessary data, the diabetes nurse educator (DNE) will reach out to you via phone or email to request the missing information. If this information is not provided in a timely fashion it may result in your child missing out on a camp plan.

    Once your camp plan has been sent you will have 48 hours to review and request any changes, after this time the plan will be considered final.

    School Action and Management plans for 2026

    All children/students with type 1 diabetes attending early childhood settings, primary or secondary schools in Victoria need to have a Diabetes Action and Management plan. The plans below have been developed by Diabetes Victoria, The Royal Children鈥檚 Hospital (RCH) and Monash Children鈥檚 Hospital, and are based on current best practice paediatric guidelines and are reviewed yearly.听 These plans are to be checked and signed by your treating team.

    If your child needs support with injections or pump data entry at school a Medication Authority Form is also required. Early Learning and Primary aged children require a Medication Authority Form.

    Steps to arrange your child's plan for Royal Children's Hospital (RCH) patients听

    1. Parents / carers complete the appropriate Diabetes School Action and Management plan (link below).听Please do not听sign the form. If you sign the PDF and a change is required, you will need to submit a new plan causing delays.

    2. After completing the form, click download and save plan as a pdf file to your computer

    3. Email diabetes@rch.org.au attaching the pdf file. Please ensure you include your child's hospital record number and if required the details for the Medication Authority Form.

    4. A Diabetes Nurse Educator (DNE) will review the plan, confirm it follows RCH diabetes treatment guidelines and, if necessary, make any changes before signing

    5. The signed plan will be returned to parents / carers.

    6. The parent/carer is then responsible for discussing and signing the plan with a school representative.

    2026 PDF Diabetes School Action and Management plans:听

    Please select the appropriate plan for your child. These plans are for early learning (creche and kindergarten) as well as the school setting (prep - year 12)听

    Multiple daily injections (requiring lunch time insulin injection)听

    Insulin pump therapy for听automated insulin Hybrid Closed Loop (Control IQ庐 Tandem T-Slim x2, SmartGuard庐 Medtronic, CamAPS庐 FX Ypso, Omnipod)听

    Insulin pump therapy, manual mode only听(not using continuous glucose monitoring).

    Other types of diabetes i.e. type 2 diabetes, cystic fibrosis related diabetes, steroid induced diabetes or monogenic diabetes:

    Requiring insulin

    Medication (not requiring insulin)

    Finally if you are unable to complete the above plan online,听click the link below for our alternate form. Note preparation of plans sent via this request takes significant DNE resources and will significantly delay your 2025 plan preparation.听

    I am unable to complete the pdf school plan above. I request a diabetes educator to complete this. Unless there is a change of insulin, this request will be completed when the 2025 plan becomes available

    Once the request is received, a DNE will complete the plan and return it to you via email if an address is provided or via post

    Medication Authority Forms (MAF) for 2026

    If school staff are required to supervise or administer insulin via injections or insulin pump therapy,听in addition to the Diabetes School Action and Management plan, a MAF听is required in addition to the Diabetes School Action and Management plan.听听

    To request a MAF please send an email to diabetesadmin@rch.org.au including the following information:听

    • Child's name and hospital UR number听
    • Name of school / kinder/Early Learning Centre听
    • Child's grade/year level听
    • Name of the insulin used at听kinder/Early Learning Centre听
    • Insulin regimen (e.g. multiple daily injections (MDI), insulin pump)
      • the approximate number of units of insulin given at lunch time
      • Is the dose of insulin determined by:听
        • Fixed dose
        • Fixed dose for food plus a correction dose听
        • Flexible dose determined using an insulin dose calculator听
        • Flexible dose determined using an insulin dosing card听
        • Insulin pump听

    Diabetes in Schools Program

    The Diabetes in Schools program is a nationally funded program, developed to provide information, training and support for schools and families to better support students living with type 1 diabetes.

    You can alert the school about the program by scrolling to the bottom of the home page and completing the 鈥淭ell a School about the program鈥 link. School principals can also request training via this website.

    You will also find听self-directed learning modules that school staff or other carers can complete to better understand type 1 diabetes听and how it impacts on a student鈥檚 day to day life.

    This program is not yet funded for childcare and kindergartens, however if your child鈥檚 early learning centre requires training they can access the Diabetes in Schools online modules at the website above
